The Qatar government has recently mandated the use of ICD 10 AM into the State of Qatar and require highly skilled people to help mentor and guide this process as we set about achieving the goals and objectives of the National Health Strategy.
POSITION SUMMARY:
Position responsibility is to bridge the gap between the physician's clinical language and coding requirements as well as undertake concurrent coding and provide immediate learning to health care providers. The roles of the clinical documentation specialists are needed to ensure that we address the inaccurate documentation and data, that ultimately reduce payments to the organization.
PRINCIPAL DUTIES:
Facilitating improvement in the overall quality, completeness and accuracy of medical record documentation by conducting concurrent coding of open records.
Concurrent coding and conducts immediate learning techniques.
Obtaining appropriate clinical documentation through extensive interaction with physicians, nursing staff, other care team members, and medical records coding staff.
Educating all members of health care providers on documentation guidelines on an ongoing basis.
Demonstrating knowledge and skills necessary to provide care appropriate to the age of the patients served.
Demonstrating knowledge of the principles of growth and development over time.
Maintaining quality admission reviews.
Completing follow-up of admission reviews.
Supporting the growth and development of care team members.
Participating in performance improvement activities.
To keep abreast of clinical coding standards, reporting, information policies and statistical trend.
Feed back to clinical coding data quality group findings and recommendations from clinical coding audits.
Monitor the completeness, timeliness and accuracy of clinical data.
Assigns appropriate codes for each diagnosis, operation and procedure based on the International Classification of Diseases and Procedures.
To achieve the corporations targets for standards of accuracy, completeness, and timeliness of clinically coded data in ICD10AM and assist Qatar as they begin the transition to ICD10AM.
Maintain and further enhance the levels of coder-clinician interaction.
To demonstrate the continued achievement of high standards of clinical coding in order to ensure that both clinicians and managers maintain a high level of confidence in the quality of the data.
To plan and develop an effective Clinical coding Audit programme and develop and implement coding strategies, policies and procedures across the Corporation
To assist the development of a clinical coding training programme for staff and mentor staff as they transition from ICD9 to ICD10AM.
To provide a source of expertise and guidance on diagnostic and procedure coding to all disciplines within the Corporation and across the health economy.
To keep abreast of clinical coding standards, reporting, and information policies according to Australian Standards and further develop standards according to Qatar healthcare requirements.
To develop and plan a Clinical Coding Audit programme involving clinician and clinical coders.
Feed back to clinical coding data quality group findings and recommendations from clinical coding audits.
Checks and analyses medical records for accuracy, adequacy and consistency of documentation.
Account for daily discharges and performs audit as necessary.
Perform other related functions.
EDUCATION:
Minimum: Degree in Health Information Technology or Medical Records.
Preferred: Master’s degree in Health Information Technology or Medical Records with certification as Registered Health Information Technician (RHIT) by the American Health Information Management Association (AHIMA) /CHIM Canadian Health Information Management or equivalent
EXPERIENCE:
Minimum: 5 years in medical records processing in ICD 10 coding. Good training background in coding and reimbursement.
Preferred: 7+ years in medical records processing in ICD 9/10 coding. Strong training background in coding and reimbursement.
